Required Forms and Additional Information
Eastern Nazarene College student-athletes must comply with all ENC, conference, and NCAA rules and regulations pertaining to meeting and maintaining athletic eligibility. Any current or prospective student-athlete must complete the following steps/forms before being cleared to participate in their first practice.
1. Health Requirements and Health Insurance
All new student-athletes will create an online health profile and submit health documentation, including a health history, a recent physical, and a current immunization record. New student-athletes entering ENC for the first time must have completed a physical examination within six months prior to arriving on campus. This is an NCAA requirement. Information on health requirements and submission procedures is available on the ENC Health and Wellness page. For any questions, contact ENC Health Services at 617-745-3892 or healthservices@enc.edu.
Massachusetts state law requires all students carry adequate health insurance that meets particular requirements to ensure that you will be taken care of in case of a health emergency. Identify whether you need to enroll in the Student Health Insurance Plan or if you are eligible to waive. The criteria for enrolling and waiving are stated on University Health Plans website. If you waive the Student Health Insurance Plan, you are responsible to determine that your insurance plan provides coverage from medical and mental health providers in the Quincy area and meets or exceeds the ENC plan. Research your options early and submit either an enrollment or waiver form online prior to arriving on campus and before the start of the semester. To review the health insurance requirements go to the University Health Plans cover page.
2. SportsWare
The Eastern Nazarene College Athletic Training staff will be using a software system called SportsWare to track each athlete's medical history and medical care. The steps to register and complete personal information can be found by clicking here. All student-athletes are required to complete this registration prior to participation, as well as, to utilize the athletic training room and their services.
3. Informational Videos
There are a number of resources available to student-athletes competing at the NCAA Division III level. While not required, you are encouraged to become familiar with the organization by viewing the videos in the “Getting in the Game” section of the NCAA website.
4. NCAA and Eastern Nazarene College Athletics Forms
In order to be cleared to participate in athletics, every student-athlete must complete a number of forms that are used in the compliance process. These forms include NCAA, ENC, and conference documents. The method used by ENC Athletics is all done electronically through online software. Each returning and prospective athlete will receive an email with instructions on how to log into the system. If you have not received this email, please contact Derek Schmitt at the email or phone number listed above. The documents include important information in addition to various fields that must be completed. All forms are then submitted electronically and stored for departmental use. The list of documents in the online system include the following:
- DIII Form 22-3c Student Athlete Statement 23-24
- DIII Form 22-3f Drug-Testing Consent 23-24
- DIII Form Summary of NCAA Regulations 23-24
